By filling out this registration form below for your child, you are agreeing to the following:
1) If your child needs emergency medical care, you give permission to the Awana Staff at Clover Hill Baptist Church to secure emergency treatment for your child.
2) You are allowing us to take and use photos of your child for Clover Hills's social media, website, and publications.
